Banking Problems

I spoke to a carer yesterday, her father (PWD) kept on attempting to withdraw money but forgetting his password. On the fifth time the bank refused to send him a card. The carer (no Power of Attorney) spoke to the bank and they suggested Third Party Mandate, at the time my father agreed. We went to the bank today and my father refused. He is not able to manage day to day affairs and I feel a bit lost. I asked my brother to speak to him and of course he agreed to everything, but I cannot be sure he will on the day?
